Golden Mask Award

The Henry County Health Department is proud to announce the Golden Mask Award! This award will be given to multiple businesses throughout Henry County based on recommendations from the county citizens. We are asking you to fill out one of the nomination forms pictured above and tell us why you think that business deserves this award and how they demonstrate excellent mask wearing.

We are open: June 15, 2020

Henry County Health Department offices are now open. The hour between 8:00 and 9:00 a.m. will be reserved for our clients who are 65 years and older and our clients with high risk medical conditions.

To protect everyone’s health we ask the you:

(1) Call ahead to the department you wish to visit. We will make every attempt to assist you remotely.

(2) Do NOT visit our office if you are feeling sick or have symptoms of COVID-19.

(3) Wear a face covering or mask when you do visit unless you have a medical condition that prevents you from wearing one.

Nursing Services

The Nursing office will continue to serve the public remotely (by phone or email) for general questions and informational requests. Our office will remain closed to walk-ins.

All immunizations, low-cost lab events, and other in-person services will be conducted by appointment only.

Vital Records Information

Office Hours: 8:00-4:00, closed for lunch 12:00-1:00

8:00 to 9:00 a.m. is reserved for our customers who are 65 and older or who are at high risk for contracting COVID-19 due to a medical condition.
Please call ahead as we may be able to assist you remotely. Certificate requests can be submitted via US mail, e-mail, or fax. Feel free to contact our office by phone if you have questions at (765) 521-7059 option 1.
IF you do need to visit our office in person, we ask that you:
1. Do NOT do so if you are feeling sick or have symptoms of COVID19.
2. Wear a face covering or mask unless you have a medical condition that would prohibit you from wearing one.

To obtain vital records such as a birth/death certificate we need the following items:

  1. Request Form
  2. Photo ID
  3. $20 per copy requested cash, check, credit/debit accepted (a convenience fee of 3% for credit/debit).

**Please note that the certificate WILL NOT be issued without the above required items**
